Make the Introduction
Fill out the referral form below with the facility's name and a brief note on their coverage needs. We'll reach out to you to learn a little more about the details of the opportunity — and if we can fill it, we'll ask you to make an email introduction to AVEA CRNA. That's all it takes.
We Go to Work
Our team reaches out, qualifies the opportunity, and — if it's a fit — gets an engagement letter in place. We handle the relationship from there. You stay in the loop, not in the weeds.
You Get Paid
Once the facility signs with AVEA, engages our services, and a placed provider hits 500 hours on assignment within a year of your referral — your $1,500 is issued. Each qualified placement earns its own bonus — up to 12 CRNAs or $18,000 per facility. We track the milestones. You just wait for the wire.

$1,500 per qualified CRNA placement for an introduction that leads to an agreement with the facility to recruit locum CRNAs — up to 12 CRNAs or $18,000 per facility. If you know more than one facility that could use reliable CRNA coverage, each referral that converts is its own bonus. There's no cap on the number of facilities you can refer — only on how many are ready to solve their staffing problem.

We believe in transparency — the same standard we hold for every contract we place. Here's exactly how the referral bonus works.
Referral Bonus: $1,500 per qualified CRNA placement, up to 12 CRNAs or $18,000 per facility.
Eligibility: Your $1,500 bonus per placement is triggered when three things happen: the facility you referred signs a contract with AVEA CRNA, engages our staffing services, and a placed provider logs 500 hours on assignment — all within one year of your referral date.
Conditions: The bonus is tied to a clean engagement. If the referred client breaches their contract, the bonus does not apply. Your referral must be submitted before AVEA has any direct contact with that facility — first introduction counts. Current AVEA clients are not eligible as referred facilities.
Payout: Once we confirm the 500-hour milestone is met, we issue payment. No chasing required on your end — we track it and reach out to you.
